If you have been involved in or witnessed an animal or pet-related incident, including an attack, bite, or injury, please complete an Animal/Pet Incident Report Form. Download the fillable PDF form, enter the required information, and save a copy for your records. Once completed, you can print the form, drop it off in person, mail it, or submit it by email to the Band Office. Providing detailed and accurate information helps the community respond appropriately and supports the safety and well-being of all residents. *NOTE: Please include photos of any injuries whenever possible.
